AHCCCS Online Application: Health-e-Arizona Plus Step by Step
Health-e-Arizona Plus is Arizona's online portal for applying to AHCCCS programs, including Medicare Savings Programs that cover the Part B premium. The system works, but it has quirks that trip up first-time users — identity verification timeouts, confusing coverage-type prompts, and document upload failures that lose paperwork without warning.
Creating a HEAplus Account
Go to healthearizonaplus.gov and register for a new account. You'll need an email address, a password, and security questions. The system verifies identity through automated data matching — if it can't confirm identity electronically, it asks for a government-issued ID upload before the application opens.
Adult children applying on behalf of a parent need their own HEAplus account. The parent should be listed as the applicant, and the adult child added as an authorized contact on the case. AHCCCS requires Form DE-301 (Authorized Representative Designation) to let a third party speak with caseworkers and receive copies of notices. Upload it with the application.
Navigating the Medicare Savings Program Path
After logging in, select "Apply for Health Insurance" from the dashboard. When the system asks which coverage types to apply for, choose "Medical Assistance."
Here's where it gets specific to Medicare beneficiaries: the application asks whether anyone in the household has Medicare. Answer yes, and enter the Medicare Beneficiary Identifier (MBI) from the red-white-and-blue Medicare card. This routes the application into the MSP screening pathway.
The system asks about assets (bank accounts, property, vehicles) because the same form covers SNAP, Cash Assistance, and full Medicaid — programs that do have asset tests. Answer truthfully, but know that the automated eligibility engine bypasses assets entirely when screening for QMB, SLMB, and QI. Arizona eliminated the asset test for all three.
Uploading Documents
HEAplus accepts document uploads in JPEG, PNG, and PDF formats. The required documents for an MSP application:
- Medicare card showing the MBI (or an SSA Benefit Verification Letter)
- Income verification — the SSA COLA award letter, pension statements, or 30 days of pay stubs
- Arizona residency — driver's license, state ID, utility bill, or lease agreement
Bank statements are not required for MSP applications because there's no asset test. If the system asks for them anyway (it sometimes does for multi-program applications), upload them to keep the file moving — they won't be used against MSP eligibility.
Upload each document immediately after completing the relevant section. HEAplus sessions can time out, and documents attached to an unsaved section may not persist.

After Submission
The system assigns a HEAplus Application ID — save it. This number establishes the legal filing date, which matters for retroactive coverage under SLMB and QI (up to 3 calendar months before the application month, as long as eligibility criteria were met; QI coverage cannot extend into a prior calendar year).
AHCCCS has 45 calendar days from receipt of the application to issue a determination. Track the status by logging into HEAplus and checking the case dashboard, or call the AHCCCS/DES Customer Service line at 1-855-HEA-PLUS.
If the portal errors out mid-application or document uploads fail, paper Form DE-103 submitted to a local DES Family Assistance office is the reliable fallback. Keep a copy of whatever was completed online — it may be partially saved in the system.
