Helping a Parent Apply for AHCCCS: Authorized Representative and Power of Attorney
You've figured out your parent qualifies for Arizona's Medicare Savings Program. The income is under the limit, they have Part A, and the $202.90 monthly Part B deduction is eating their Social Security check. So you call AHCCCS to ask about their case — and the representative tells you they can't discuss anything without authorization on file.
This is one of the most common dead ends adult children hit when managing a parent's benefits in Arizona.
The Form You Need: DE-301
AHCCCS requires Form DE-301 (Authorized Representative Designation) to allow a third party to act on a beneficiary's behalf. Once filed, the authorized representative can:
- Speak with DES caseworkers about the application or case status.
- Receive copies of all official notices.
- Submit documents, respond to verification requests, and manage the application file.
Your parent signs the DE-301 designating you as their representative. If they can still sign documents but struggle with the bureaucratic process — which describes most families in this situation — the DE-301 is the fastest path.
When Power of Attorney Works (and When It Doesn't)
A general Financial Power of Attorney is accepted by AHCCCS only if it explicitly grants authority to manage public benefits. Many boilerplate POA documents cover banking and real estate but say nothing about government assistance programs. If yours doesn't mention public benefits, AHCCCS can reject it.
Also critical: SSA representative payee status and federal SSA-1696 forms are not automatically recognized by AHCCCS. These are federal designations. Arizona's state agencies require state-specific documentation. Having POA with Social Security does not mean you can talk to DES about an AHCCCS application.
Don't Share HEAplus Login Credentials
If you're submitting the application through Health-e-Arizona Plus, create the account in your parent's name using their information. Under state administrative standards, HEAplus account passwords should not be shared with third parties.
In practice, many adult children set up the account and handle the data entry themselves, which works fine for submission. But if there's ever a dispute about the application or an appeal, having the DE-301 on file is what gives you legal standing — not access to the portal.

Managing the Application from Out of State
Many adult children managing a parent's care in Arizona don't live there themselves. Much of the state application can be handled remotely:
- HEAplus accepts online applications and document uploads from any location.
- Form DE-103 (the paper application) can be mailed or faxed to a local DES Family Assistance Administration office.
- DE-301 requires your parent's signature. Ask DES how to submit the signed form.
The challenge is that DES caseworkers sometimes call the applicant (your parent) directly with questions. If your parent has cognitive difficulties or hearing loss, they may not answer or may give confused responses. Having the DE-301 on file means you can call back on their behalf, but you need to check the case regularly rather than waiting for AHCCCS to reach out.
The Two-Agency Problem
Medicare Savings Programs involve both AHCCCS (state) and Social Security (federal). Authorization with one doesn't carry over to the other. If you also need to contact SSA about your parent's Part B premium deduction or buy-in status, you'll need separate federal authorization. Contact SSA to confirm which authorization applies to the specific issue.
Plan for both authorizations at the same time. The buy-in delay after AHCCCS approval (60 to 90 days before Social Security stops the premium deduction) is the phase where you'll need to contact SSA directly.
The Arizona Medicare Savings Programs & Extra Help Guide walks through both the state and federal authorization processes step by step, including what to say on the phone when agencies push back, and a communication log template for tracking every call across both systems.
