Virginia QMB vs Medicaid: What's the Difference?

They're Both Medicaid — But Very Different

QMB, SLMB, and QI are limited-benefit Medicaid categories. You apply through the same local DSS office, your approval letter says "Medicaid" on it, and you may receive a Cardinal Care (Virginia Medicaid) card. But these programs cover something very specific: your Medicare costs, not the full range of medical services that comprehensive Medicaid provides.

The distinction matters because the benefits, provider networks, and even estate recovery rules differ significantly between MSP-only coverage and full Medicaid.

What Each Program Covers

QMB pays your Medicare Part A premium (if applicable), Part B premium ($202.90/month in 2026), Part A and Part B deductibles, and all coinsurance and copays on Medicare-covered services. You also get federal balance-billing protection — providers cannot bill you for any Medicare cost-sharing. QMB does not, by itself, cover dental, vision, hearing aids, or long-term care.

SLMB pays only the Part B premium. No deductibles, no coinsurance, no copays. It reduces your monthly costs by $202.90 but doesn't touch out-of-pocket expenses at the doctor or hospital.

QI also pays only the Part B premium, funded through a separate federal block grant. QI is mutually exclusive with full Medicaid — you cannot have both simultaneously.

Full Medicaid (Aged, Blind, or Disabled categories) covers a comprehensive range of services: doctor visits, hospital stays, prescription drugs, home health, nursing facility care, dental, vision, mental health, transportation, and more — depending on the specific coverage group.

When You Can Have Both

If a QMB or SLMB enrollee's income falls below 80% FPL — or if they qualify through Virginia's medically needy spenddown — they can receive full Medicaid alongside the Medicare Savings Program. This combination creates what's called a full-benefit dual eligible (FBDE).

As an FBDE, Medicare remains the primary payer for Medicare-covered services, QMB handles the cost-sharing on those services, and full Medicaid fills in everything else: prescription drugs (through Medicaid rather than Part D in some cases), dental, transportation, and potentially long-term care.

QI is the exception — it cannot coexist with full Medicaid. If a QI enrollee becomes eligible for full Medicaid, the local DSS must terminate QI and transition them to full Medicaid, possibly with SLMB layered on top.

Provider Networks: Not a Managed Care Plan

A common fear is that applying for QMB or any MSP will force a switch away from current doctors to a managed care network. It won't. Medicare Savings Programs are financial subsidies, not insurance plans. Enrollees keep their existing Medicare coverage — whether that's Original Medicare or a Medicare Advantage plan — and see the same providers.

The Medicare Savings Program doesn't restrict where you go for care. It just pays your share of the bill.

D-SNP vs QMB

Dual-Eligible Special Needs Plans (D-SNPs) are Medicare Advantage plans designed for people who have both Medicare and Medicaid. They're private insurance products with defined provider networks, and they often advertise benefits like dental and vision.

QMB is a state subsidy, not an insurance plan. You don't have to enroll in a D-SNP to get QMB benefits. If your parent is happy on Original Medicare with their current doctors, QMB works perfectly alongside that arrangement. D-SNPs are an option, not a requirement — and for many QMB beneficiaries, remaining on Original Medicare with QMB cost-sharing protection is the simpler path.
