Virginia QMB Approved but Social Security Still Deducting Part B

Why the Deductions Continue

You received the approval letter from your local Virginia DSS. The letter says QMB coverage is active. But the next Social Security check still shows $202.90 deducted for the Part B premium. And the one after that.

This isn't a mistake in your application. It's a timing gap built into how the system works.

After your local DSS approves your QMB application, the approval record has to travel through three separate systems: from the local DSS to the DMAS central buy-in clearinghouse, from DMAS to the CMS Third Party Liability database, and from CMS to SSA's payment records. Only after SSA's system reflects the state buy-in does the premium deduction stop.

This reconciliation loop typically takes 30 to 90 days. During that time, Social Security continues deducting the Part B premium from your monthly check as though nothing has changed.

You Will Get the Money Back

Every dollar deducted after your QMB effective date will be refunded. SSA issues a lump-sum check for all the overpaid premiums once the buy-in records sync. If your QMB effective date was November 1 and SSA stops deducting in February, you'll receive a refund covering the November, December, and January premiums — roughly $609 at the 2026 rate.

The refund check comes directly from SSA, separate from your regular monthly benefit deposit.

What to Do in the Meantime

Don't panic. The continued deductions do not mean your QMB was denied or isn't working. Your approval letter from Virginia DSS is the official record.

Track each deduction. Note the date and amount of every Part B premium deducted after your QMB effective date. When the refund arrives, compare it against your records to confirm the full amount was reimbursed.

Call SSA after 60 days. If deductions are still occurring more than two months after your QMB effective date, call Social Security at 1-800-772-1213. Ask them to verify whether they've received the state buy-in notification from Virginia DMAS. Sometimes the electronic file transmission gets stuck, and a phone call prompts someone to check.

Call DMAS if SSA says no notification. If SSA has no record of the buy-in, contact DMAS at 804-786-7933 to verify the buy-in file was transmitted. The DMAS clearinghouse sends monthly data files to CMS — your record should be in the batch following your approval.

SLMB and QI Work the Same Way

The premium reimbursement process is identical for SLMB and QI enrollees. The only operational difference is that SLMB and QI can have retroactive effective dates (up to three months before the application), while QMB starts the month after approval. For retroactive periods, SSA refunds all premiums deducted during the covered retroactive months as well.

A Separate Issue: The Approval Month Gap

Because QMB coverage can't be backdated, there's always at least one month — the approval month itself — where the applicant has no MSP coverage. During that gap month, DMAS recommends applying for Extra Help at SSA immediately so prescription drug coverage starts right away, even before QMB kicks in the following month.

The Virginia Medicare Savings Programs & Extra Help Guide includes a premium refund tracking log and the exact call scripts for both SSA and DMAS to resolve delayed reimbursements.
