Utah Part A Group Payer: How to Get Medicare Without 40 Work Quarters
Your parent is 66, has 25 work quarters, and just found out Medicare Part A will cost them $505 a month. On a $900 Social Security check, that's not happening. Utah has a specific mechanism for this.
Utah Is a Group-Payer State
Utah is one of the states designated as a Part A Buy-In / Group-Payer state under SSA's Program Operations Manual (POMS HI 00801.140 and HI 00815.023). This means low-income Utah residents who lack premium-free Part A can use the QMB program to have the state pay their Part A premiums.
The Part A premium in 2026 is $505/month for individuals with fewer than 30 work quarters, or $278/month for those with 30–39 quarters. QMB covers the full premium regardless of the amount.
The Conditional Enrollment Process
The catch: you can't just apply for QMB at DWS and expect Part A to appear. Your parent needs Part A entitlement first — but they can't afford to actually pay the premium. The solution is a conditional enrollment sequence that coordinates SSA and DWS:
Step 1: File conditional Part A enrollment at SSA During the General Enrollment Period (January 1 through March 31 each year), your parent visits a local SSA field office and files an application for Medicare Part A. They explicitly select "Conditional Enrollment," which means Part A coverage doesn't take effect — and premiums don't start — unless QMB is subsequently approved by Utah DWS.
SSA issues a document confirming the conditional application.
Step 2: Submit MSP application to DWS Take the SSA conditional enrollment confirmation to DWS. File an MSP application through myCase, by mail (Form 61), or in person at a DWS Employment Center. Include the SSA document as part of the application package.
Step 3: DWS evaluates and approves QMB If your parent's income is at or below $1,350/month (single) or $1,824/month (couple) and resources are within limits, DWS approves QMB. The approval is transmitted to CMS, which triggers state payment of the Part A premium.
Step 4: Part A activates Once CMS processes the buy-in, your parent has Medicare Part A without ever paying the premium out of pocket. The state pays it going forward through QMB.
If SSA Pushes Back
Some SSA field staff are unfamiliar with conditional enrollment and may tell your parent they can't apply for Part A without paying the first premium. This is incorrect. If this happens, ask to speak with a supervisor and cite POMS HI 00801.140 — "Premium-Part A Enrollments for Qualified Medicare Beneficiaries." That's the internal SSA reference that governs this process.

Timing Matters
The General Enrollment Period is the only window for conditional Part A enrollment: January 1 through March 31. If your parent misses it, they wait until the next year. Under current Medicare GEP rules, premium-Part A coverage starts the month after enrollment, not automatically on July 1. Ask SSA how QMB approval affects the effective date for a conditional application.
The Special Enrollment Period may apply in specific circumstances (loss of employer coverage, for example), but for the conditional-enrollment-plus-QMB path, GEP is the standard route.
Who This Applies To
This matters most for:
- Naturalized citizens with fewer than 40 quarters of U.S. work history
- Individuals who spent most of their career in work not covered by Social Security (some government, some nonprofit, some foreign employment)
- Surviving spouses who don't qualify for premium-free Part A on their own or their deceased spouse's record
What QMB Covers Beyond Part A
Once QMB is approved, it doesn't just pay Part A premiums. It also covers the Part B premium ($202.90/month in 2026), plus all Medicare deductibles, coinsurance, and copays. And it automatically triggers Part D Extra Help for prescription drug savings.
The Utah Medicare Savings Programs & Extra Help Guide covers the conditional enrollment process step by step, including what to bring to the SSA field office, how to document the conditional filing for DWS, and what to do if the first attempt is refused.
