What TENS Is and How It Works

Transcutaneous Electrical Nerve Stimulation (TENS) delivers low-voltage electrical currents through electrode pads placed on the skin near painful areas. The electrical pulses work through two mechanisms: they stimulate the nerves in a way that blocks pain signals from reaching the brain (the gate control theory), and at certain frequencies they trigger the release of endorphins — the body's natural pain-relieving chemicals.

For older adults with chronic joint pain, muscle stiffness, or neuropathic pain, TENS offers a drug-free option that may be used at home after appropriate setup and contraindication review. The units are portable, battery-operated, and available without a prescription in most countries for $25 to $80.

But TENS comes with a critical safety issue for older adults that most product packaging barely mentions, and its clinical acceptance varies dramatically depending on which country's guidelines your parent's doctor follows.

The Pacemaker Warning: This Is Not Optional

TENS is absolutely contraindicated in patients with cardiac pacemakers, implantable cardioverter-defibrillators (ICDs), or any other active implanted electronic device. This isn't a theoretical concern — applying electrical currents near these implants can cause device malfunction, cardiac arrhythmias, or fatal cardiac events.

Before using a TENS unit on any older adult, you must confirm they don't have an implanted cardiac device. This isn't always obvious — not all patients wear medical alert bracelets, and some forget to mention their pacemaker because it was implanted years ago and "doesn't cause problems." Check their medical records or ask their cardiologist directly.

Other safety precautions:

  • Don't place electrodes over the carotid sinus (front of the neck) — risk of blood pressure drop and loss of consciousness
  • Don't use on skin with open wounds, infections, or active dermatitis
  • Don't apply over the abdomen or pelvis during pregnancy
  • Use caution in patients with epilepsy — some frequencies can trigger seizures

Where to Place the Electrodes

Electrode placement determines whether TENS helps or wastes your parent's time. The general principle is to bracket the painful area — place one pad on each side of the pain site, roughly 2 to 4 inches apart.

For knee osteoarthritis: Place two electrodes on either side of the kneecap, on the inner and outer aspects of the joint. A second pair can be placed above and below the knee if pain radiates along the thigh or shin.

For lower back pain: Place electrodes on either side of the spine, at the level of the most intense pain. Never place electrodes directly over the spine itself.

For shoulder pain: Place on the front and back of the shoulder, avoiding the neck area.

For hand or wrist arthritis: Place on either side of the affected joint. The smaller pads designed for hands and feet work better than standard-size electrodes.

Start at the lowest intensity setting and increase gradually until your parent feels a strong but comfortable tingling sensation. The sensation should not be painful. Follow the device instructions or clinician's advice for session length and frequency.

The International Policy Split on TENS

This is where TENS gets complicated — clinical guidelines disagree sharply on whether it works:

United States: TENS is used as an adjunctive, non-invasive pain management option under clinical parameters and after contraindications are reviewed.

United Kingdom (NICE): NICE Guideline NG193 formally recommends against offering TENS for chronic primary pain, citing insufficient high-quality evidence. This means NHS clinicians are unlikely to recommend it, though patients can purchase units privately.

Canada: Evaluated as an acceptable adjunctive physical therapy method. Supported by national evidence summaries and used in clinical practice.

Australia: Supported as a self-management tool within comprehensive multidisciplinary physical therapy protocols under the National Strategic Action Plan for Pain Management.

What does this mean practically? If your parent's doctor is following NICE guidelines (common in the UK and countries influenced by UK practice), they may advise against TENS. If the doctor follows US or Australian guidelines, they're more likely to support a trial. Neither position is wrong — the evidence is genuinely mixed, with TENS showing benefits in some studies and no significant effect in others.

The pragmatic approach: TENS is inexpensive, but it should be treated as a clinician-guided trial after contraindications are reviewed. Some older adults report meaningful relief even if the clinical trial data is equivocal. A two-week trial — using the unit consistently, at the right intensity, with proper electrode placement — gives you and your parent concrete evidence about whether it works for them specifically.

Practical Setup Tips for Caregivers

Skin preparation matters. Clean the skin with mild soap and water before applying electrodes. Dry thoroughly. Elderly skin often has lotions, oils, or residual medications (from patches or topical creams) that reduce electrode adhesion and conductivity.

Gel electrode pads have a limited lifespan. Replace them when they no longer adhere properly or when the device instructions say to do so. Poor adhesion can cause skin irritation or mild burns.

Start with a supervised session. The first time your parent uses TENS, stay with them for the full session. Watch for skin reactions, ensure the intensity is comfortable, and make sure they know how to turn it off if the sensation becomes unpleasant.

Establish a routine. TENS works best when used consistently as part of a daily pain management plan — for example, a session before physical activity if the clinician or device instructions support that schedule. Occasional, irregular use is less likely to produce meaningful results.

Track the results. Add TENS sessions to your pain diary: note the time, duration, electrode placement, and pain scores before and one hour after. Two weeks of data will clearly show whether TENS provides your parent with measurable relief or whether you should redirect effort to other non-drug approaches.

When TENS Is Worth Trying

TENS is most likely to help older adults with:

  • Chronic osteoarthritis pain in specific joints (knees, hands, shoulders)
  • Chronic lower back pain
  • Peripheral neuropathic pain (in the absence of implanted devices)
  • Pain that responds to distraction or counter-stimulation (some people's nervous systems are more responsive to gate-control mechanisms)

It's less likely to help with:

  • Widespread fibromyalgia-type pain (the localized nature of electrode placement limits coverage)
  • Pain that's primarily inflammatory (cold therapy may be more appropriate)
  • Deep visceral or organ pain
