TBI SCI Waiver Mississippi

Mississippi's home-based Medicaid programs include the Elderly and Disabled (E&D), Independent Living (IL), and Traumatic Brain Injury / Spinal Cord Injury (TBI/SCI) waivers, each targeting a different population. The TBI/SCI Waiver is the most specialized of these — designed for individuals whose primary disability stems from a traumatic brain injury or a spinal cord injury, rather than general aging or physical disability.

Who Qualifies

The TBI/SCI Waiver has clinical eligibility requirements that are distinct from both the Elderly and Disabled (E&D) Waiver and the Independent Living (IL) Waiver:

• Diagnosis requirement: The applicant must have a documented traumatic brain injury or spinal cord injury as the primary disabling condition. Degenerative neurological conditions (like Parkinson's or ALS) and age-related decline do not qualify under this waiver — those fall under the E&D or IL programs.
• Age: The TBI/SCI Waiver carries no age restriction; eligibility also depends on the applicant's Medicaid category and the waiver's clinical criteria.
• Level of care: The applicant must meet Mississippi's Nursing Facility Level of Care (NFLOC) standard — the same clinical threshold used for the E&D Waiver, confirmed through the LTSS/InterRAI assessment.
• Financial eligibility: The applicant must qualify for Medicaid in an eligible category; Mississippi lists several possible categories, including income up to 300% of the SSI federal benefit rate. Financial rules depend on the eligibility category. If income exceeds the applicable cap, a Qualified Income Trust may be required.

Services Covered

The TBI/SCI Waiver covers case management, attendant care, respite, environmental accessibility adaptations, specialized medical equipment and supplies, and transition assistance. It includes services that the E&D Waiver explicitly excludes:

Environmental Accessibility Adaptations. This is the major differentiator. The E&D Waiver covers only minor environmental safety services (cleaning, pest control, minor maintenance). The TBI/SCI Waiver pays for structural home modifications: wheelchair ramp installations, doorway widening, roll-in shower conversions, grab bar mounting, and other physical changes to the home that are necessary for the participant to live safely in the community. Each modification must be pre-approved and is subject to state-approved dollar caps.

Specialized equipment and transition assistance. Depending on the participant's service plan, the waiver may authorize specialized medical equipment and supplies or transition assistance — services that go beyond the personal care and homemaker assistance the E&D Waiver provides.

How It Compares to Other Mississippi Waivers

The IL Waiver and TBI/SCI Waiver both cover home modifications, but they serve different diagnostic populations. If your parent's disability is from a spinal cord injury sustained in a fall or accident, the TBI/SCI Waiver is the correct program. If the disability is from a degenerative condition like arthritis or age-related mobility loss, the E&D Waiver is generally the relevant starting point; the IL Waiver is for severe orthopedic or neurological impairments.

How to Apply

The application process follows the same general path as other HCBS waivers:

1. Contact your local Planning and Development District (PDD) or call the Mississippi Access to Care helpline at 1-844-822-4622. Tell them specifically that you are inquiring about the TBI/SCI Waiver — the intake specialist needs to route the assessment to the correct waiver program.
2. Clinical assessment. A PDD case management team (registered nurse and licensed social worker) conducts the in-home LTSS assessment to confirm NFLOC eligibility.
3. Financial screening. The Division of Medicaid verifies income and asset eligibility through the regional office.
4. Service plan development. Once approved, the case management team develops an individualized service plan specifying the authorized services, home modifications, equipment, and transition assistance.
