A fingertip pulse oximeter costs less than a restaurant meal and gives you one of the most important vital signs in real time: blood oxygen saturation (SpO2). For caregivers of elderly parents with heart failure, COPD, pneumonia risk, or any respiratory condition, it's arguably the single most useful home monitoring tool after a thermometer.

But the numbers mean nothing if you don't know what they indicate — or when the device is giving you a false reading.

What the Numbers Mean

A pulse oximeter displays two values:

SpO2 (oxygen saturation): The percentage of hemoglobin molecules carrying oxygen. This is the number that matters most.

  • 95-100% — Normal for most healthy adults. People with heart or lung problems may have lower clinician-set targets.
  • 90-94% — Below the usual healthy range. Repeat the reading according to the device instructions and contact your parent's care team, especially if this is new or falling. For COPD patients, use the target set by their care team rather than a generic range.
  • Below 90% — A concerning reading that should be repeated promptly and discussed with the care team. If your parent shows symptoms such as shortness of breath, confusion, or blue lips, call 911.

Pulse rate: Beats per minute. Normal resting range for older adults is 60-100 bpm. A pulse oximeter displaying a heart rate above 100 at rest, or below 50, alongside a low SpO2, adds urgency.

When to Call 911 Based on Oximeter Readings

Use the oximeter reading alongside clinical observation — the number alone isn't enough:

Call 911 if:

  • SpO2 below 90% with shortness of breath, confusion, or cyanosis (blue/gray lips or fingertips)
  • SpO2 dropping rapidly — a reading that falls substantially over minutes may signal a serious change and needs prompt medical attention
  • Any reading accompanied by severe respiratory distress (gasping, inability to speak, tripod positioning)

Call the doctor same day if:

  • A low reading without symptoms still needs prompt clinician guidance because some people have few or no symptoms of low oxygen
  • SpO2 consistently 90-94% at rest when their baseline is normally 95+
  • SpO2 drops below 90% with activity but recovers at rest
  • Oxygen readings are trending downward over days

Common Sources of False Readings

Pulse oximeters are reliable in most conditions, but several factors can produce inaccurate readings in elderly patients:

Cold hands or poor circulation — The oximeter needs pulsatile blood flow to measure. Cold or poorly perfused fingers can make readings less reliable. Warm the hand first or try a different finger.

Nail polish or artificial nails — Dark-colored polish (especially blue, black, or green) can interfere with the light wavelengths. Remove polish from at least one finger or use the earlobe with an appropriate clip.

Motion artifact — Tremors (common in Parkinson's) or restless movements produce erratic readings. Steady the hand and wait for the reading to stabilize for 10-15 seconds.

Peripheral edema — Severely swollen fingers can impair signal quality. Try the earlobe or a less edematous finger.

Anemia — Severe anemia can produce normal SpO2 readings despite inadequate oxygen delivery. The oximeter measures the percentage of hemoglobin carrying oxygen, not the total amount of hemoglobin. A severely anemic patient can show 98% saturation while their tissues are starved.

Skin pigmentation — Pulse oximeters can be less accurate in people with darker skin tones, particularly at lower oxygen levels. Interpret the reading with symptoms and clinical guidance rather than trying to calculate an exact correction.

How to Use It Correctly

  1. Rest for 5 minutes before measuring — exertion skews readings
  2. Warm the hand if it feels cold
  3. Place the sensor on a clean, dry finger — middle or index finger typically gives the strongest signal
  4. Hold still and wait until the reading stabilizes — don't grab the first number
  5. Note the pulse rate waveform if the device shows one — a stable waveform can support signal quality, but it does not guarantee an accurate reading
  6. Record the reading with date, time, and any symptoms

Know Your Parent's Baseline

A single reading is minimally useful. Tracking establishes your parent's normal range, which makes any deviation immediately recognizable. Some parents normally run 96-98%. Others with chronic lung disease may have lower clinician-set targets, including when using supplemental oxygen. Without knowing the baseline, you can't assess change.
