NJ Medicaid Managed Care Organizations: How to Choose Your MLTSS Plan

Why MCO Selection Matters

Once your parent is approved for Medicaid long-term care in New Jersey, the next step is choosing a Managed Care Organization (MCO). This is the health plan that will coordinate all of your parent's medical care, personal care, and long-term services under MLTSS. The MCO assigns a care manager, authorizes services, and manages the provider network.

Picking the wrong MCO can mean your parent's preferred nursing home or home care agency isn't in network, the care manager is unresponsive, or needed services take weeks to authorize. It's one of the most consequential decisions in the process — and families often rush through it because it comes at the end of a grueling application.

The Five MCOs in New Jersey

New Jersey contracts with five MCOs to deliver MLTSS services:

  1. Horizon NJ Health — the largest MCO in the state, with the broadest provider network. Strong coverage in northern and central New Jersey. Operated by Horizon Blue Cross Blue Shield.

  2. Wellpoint (formerly Amerigroup New Jersey) — strong presence statewide, particularly in urban areas. Part of Elevance Health (formerly Anthem).

  3. UnitedHealthcare Community Plan — extensive national infrastructure with a large New Jersey network. Known for technology tools (member portal, mobile app for care coordination).

  4. Aetna Better Health of New Jersey — solid network across the state, part of CVS Health. Often competitive in home care and assisted living provider availability.

  5. Fidelis Care — originally a New York–based plan, now operating in New Jersey. Smaller network footprint compared to Horizon and UnitedHealthcare.

All five MCOs are required to cover the same core MLTSS benefits — nursing home care, assisted living, home care, adult day care, personal care assistance, and durable medical equipment. The differences are in their provider networks, care management quality, and supplemental benefits.

How to Choose

Check the provider network first. If your parent is already in a nursing home or uses a specific home care agency, verify that the facility or agency participates in the MCO's network. Each MCO publishes a provider directory — call the MCO directly or check their website. If the current provider isn't in network, your parent may need to switch facilities or agencies.

Ask about care manager responsiveness. The care manager is your primary point of contact for authorizing services, resolving issues, and adjusting the care plan. Some MCOs assign care managers proactively; others require you to call in. Ask the MCO: how often will the care manager contact us? How do we reach them in an emergency?

Look at supplemental benefits. Beyond the required MLTSS coverage, some MCOs offer extras: transportation to medical appointments, over-the-counter health products, dental or vision coverage beyond Medicaid minimums, or caregiver support programs.

Consider geographic coverage. If your parent might move between counties or transition from home care to a facility in a different area, check that the MCO has adequate providers in both locations.

You're not locked in permanently. If you need to change MCOs, ask NJ FamilyCare whether an open-enrollment or for-cause change is available for your circumstances and when it would take effect.

Don't Rush This Decision

MCO enrollment happens after financial and clinical approval, when families are exhausted from the application process. Take a few days to compare networks and ask questions. The MCO choice shapes your parent's daily care experience — who shows up, how quickly services are authorized, and how problems get resolved.
