New Mexico Medicare Costs 2026: Programs That Reduce What Seniors Pay
What Medicare Costs in 2026
Standard Medicare costs in 2026 include:
| Cost | 2026 Amount |
|---|---|
| Part B premium | $202.90/month |
| Part A premium (without premium-free eligibility) | $311/month if the applicant or spouse has 30–39 qualifying quarters; $565/month if neither record has 30 |
| Part A hospital deductible | $1,736 per benefit period |
| Part B annual deductible | $283 |
| Part B coinsurance | 20% of approved amounts |
| Part D prescription out-of-pocket cap | $2,100/year |
For a senior living on $1,200/month in Social Security, the Part B premium alone takes nearly 17% of their income before they've paid for groceries, housing, or prescriptions.
Medicare Savings Programs: The First Line of Defense
New Mexico's three main Medicare Savings Programs operate on income alone — no asset test since 2021:
QMB (up to $1,330/month single): Pays Part A premium, Part B premium, all deductibles, and all coinsurance. Prohibits doctors from billing you for the 20% Medicare doesn't cover.
SLMB ($1,330–$1,596/month single): Pays Part B premium. That's $202.90/month back in your Social Security check.
QI ($1,596–$1,796/month single): Same as SLMB — pays Part B premium for a slightly higher income range.
The $20 unearned income disregard means someone earning $1,350 in gross Social Security income qualifies for QMB (countable income: $1,330). Couple limits are higher: $1,804 for QMB, $2,164 for SLMB, $2,435 for QI.
All three programs automatically trigger Extra Help with Part D prescription drug costs.
Extra Help with Prescriptions
For seniors spending heavily on medications, Extra Help (the Part D Low-Income Subsidy) covers Part D plan premiums up to the benchmark amount, eliminates the annual deductible, and reduces copays to a few dollars per prescription.
There's no separate application if you're enrolled in an MSP — it happens automatically through electronic data exchange between the state and CMS.
New Mexico has no State Pharmacy Assistance Program, making the MSP-to-Extra Help pathway the primary route to affordable prescriptions for low-income Medicare beneficiaries.

Free Counseling and Navigation
New Mexico's SHIP (State Health Insurance Assistance Program) provides free, one-on-one Medicare counseling with no insurance sales agenda. Counselors are available through the Aging and Disability Resource Center:
- Phone: 1-800-432-2080 (ALTSD/ADRC)
- Services: Plan comparisons, MSP application assistance, billing dispute guidance
SHIP counselors can help identify which programs your parent qualifies for and assist with the application process. They can't file applications on your behalf or track cases in the portal, but they can intervene with the Income Support Division when applications stall.
The Social Security Fairness Act Impact
Seniors who worked in public service (teachers, firefighters, police) may have seen their Social Security checks increase after SSA began paying retroactive adjustments in February 2025. The repeal applies to benefits payable from January 2024 onward, and implementation is complete. Higher monthly income may have shifted them from one MSP tier to another — from QMB to SLMB, for instance — but the programs are designed to accommodate these transitions. The one-time retroactive lump sum is treated as income only in the month received; any unspent balance becomes a resource afterward, which New Mexico disregards for MSP eligibility.
Next Steps
The New Mexico Medicare Savings Programs & Extra Help Guide covers the full application process, income calculation worksheets, and post-approval tracking for all three programs. It includes everything from initial eligibility screening through renewals, billing disputes, and fair hearings.
