What the Central Registry Is
The Central Registry is New Mexico's statewide waiting list for Medicaid-funded home and community-based services under the Turquoise Care program. Unlike nursing facility care — which is an entitlement with no waitlist for eligible applicants — the Community Benefit waiver that funds home care has a limited number of enrollment slots allocated by the state each year.
When the available slots are full, new applicants go on the Central Registry. The Aging and Long-Term Services Department (ALTSD) manages the list through the Aging and Disability Resource Center (ADRC), reachable at 1-800-432-2080.
Being on the Central Registry does not mean your parent is enrolled in Medicaid or receiving any services. It simply means they are in line for a waiver allocation. When a slot opens, the state contacts the next person on the list to begin the formal enrollment process.
How to Get on the Central Registry
Call the ADRC at 1-800-432-2080 and request a long-term care clinical registry intake. The intake worker collects basic demographic information, the applicant's functional limitations, and contact details for the primary caregiver or authorized representative.
No financial eligibility determination happens at this stage. You do not need bank statements, tax returns, or a Qualified Income Trust to get on the list. The purpose of the registry intake is to establish your parent's place in line, not to process a Medicaid application.
That said, the applicant must eventually meet the same clinical and financial standards required for nursing facility Medicaid — a Nursing Facility Level of Care determination (requiring assistance with at least two Activities of Daily Living), countable assets at or below $2,000, and gross monthly income at or below $2,982 (or a properly funded Qualified Income Trust if income exceeds the cap).
What Happens While You Wait
Wait times vary widely depending on the region and the number of available slots. During the wait, your parent receives no Medicaid-funded home care services through the Community Benefit.
Families in this limbo face a hard choice: pay for private home care out of pocket, provide informal caregiving, or consider nursing facility placement (which has no waitlist for Medicaid-eligible individuals). Many families burn through savings on private-duty home care while waiting for a waiver slot, inadvertently spending down to the $2,000 asset limit without the legal protections and documentation that a structured spend-down provides.
Use the waiting period productively. Gather the 60 months of financial records the Income Support Division will require when a slot opens. Establish a Qualified Income Trust if your parent's income exceeds $2,982. Record a Transfer on Death deed on the family home. These steps cannot wait until a slot opens — you will need the records and trust in place when enrollment is offered, and gaps in documentation can delay approval.

When a Slot Opens
The ADRC contacts the applicant or their authorized representative when a Community Benefit enrollment slot becomes available. At that point, the applicant must:
- Select one of the four Turquoise Care MCOs (BCBSNM, Presbyterian, Molina, or UnitedHealthcare) by calling 1-888-997-2583
- Complete a Comprehensive Needs Assessment through the selected MCO
- File a formal Medicaid application through YesNM or the ISD office
- Meet all financial and clinical eligibility requirements
The applicant enters Agency-Based Community Benefit (ABCB) services first, with a mandatory 120-day period before they can transition to Self-Directed Community Benefit (SDCB) if they prefer to hire and manage their own caregivers.
