Maine Medicare Savings Program Income Limits 2026

Maine Runs One of the Most Generous Medicare Savings Programs in the Country

Most states cap QMB eligibility at 100% of the Federal Poverty Level — roughly $1,330 a month for a single person in 2026. Maine's legislature raised that ceiling to 185% FPL under 22 M.R.S. § 3174-LLL. The practical result: a single Maine resident with countable income up to $2,461 per month can qualify for QMB, which pays the full Part B premium ($202.90/month in 2026), covers Part A premiums if owed, and eliminates Medicare deductibles and coinsurance.

Maine also absorbed the federal SLMB category entirely into QMB. In other states, people between 100% and 120% FPL qualify only for SLMB, which covers just the Part B premium. In Maine, those same individuals get full QMB benefits — cost-sharing protections included.

2026 Income Limits by Program and Household Size

Program Individual Countable Monthly Income Couple Countable Monthly Income FPL Basis
QMB $2,461 $3,337 185% FPL
QI $3,325 $4,509 250% FPL

QMB covers the Part B premium, Part A premium (when owed), and all Medicare deductibles and coinsurance. QI covers only the Part B premium — but reaches people with higher incomes. Both programs run through Maine DHHS Office for Family Independence.

OFI evaluates gross monthly income and applies Maine's statutory disregards before comparing countable income with these published ceilings.

Maine Eliminated the Asset Test

Since January 1, 2024, Maine applies no asset or resource test for any MSP category. Bank accounts, retirement funds, vehicles, real estate — none of it counts. Eligibility is determined entirely by monthly income.

This is a significant departure from the federal baseline, where standalone Extra Help applies a countable-resource limit. In 2026, the limit is $16,590 for an individual and $33,100 for a couple, or $18,090 and $36,100 when the applicable burial exclusion is included. People who get denied Extra Help through SSA because of savings can apply for QMB or QI through Maine DHHS instead — and the MSP approval automatically triggers deemed Extra Help eligibility, bypassing the federal resource test entirely.

How Income Is Counted

OFI evaluates gross monthly income using SSI-related (Non-MAGI) methodology:

  • Counted: Social Security (OASDI/SSDI), private pensions, VA pensions, annuities, gross wages, regular trust distributions
  • Disregarded: $20 per month general unearned income exclusion, $65 plus 50% of remaining earned income, spousal allocations up to $484/month
  • One-time payments: Lump sums (insurance settlements, retroactive benefit adjustments) count only in the month received — not projected forward

An adult child's income is never deemed to the parent for MSP purposes, even if they live together and claim the parent as a tax dependent.

Maine's Expanded Thresholds vs. Federal Baseline

Parameter Federal Baseline Maine
QMB income ceiling 100% FPL (~$1,330/mo) 185% FPL ($2,461/mo)
QI income ceiling 135% FPL (~$1,816/mo) 250% FPL ($3,325/mo)
SLMB Separate category (120% FPL) Absorbed into QMB
Asset test $9,950 single / $14,910 couple None

National Medicare guides, including materials published by CMS, still show the federal baseline numbers. A Maine resident reading those figures would incorrectly conclude they earn too much to qualify. Always use the Maine-specific thresholds above.

QMB Approval Also Triggers Extra Help and DEL

Getting approved for QMB or QI in Maine sets off a cascade of additional benefits:

  1. Extra Help (Part D LIS): Automatic deemed eligibility — no separate SSA application needed. Eliminates the Part D deductible, caps generic copays at $5.10 and brand-name at $12.65 in 2026.
  2. DEL (Low Cost Drugs for the Elderly or Disabled): Maine's own pharmacy assistance program. If you meet DEL's 185% FPL income limit (same as QMB), enrollment is automatic when you're approved for MSP. DEL wraps around Part D, capping out-of-pocket drug costs at $2.00 plus 20% of the allowed cost.

What to Do Next

If your parent's gross monthly income is near these thresholds, the statutory disregards may bring countable income within them; an MSP application is worth filing. The Maine Medicare Savings Programs guide walks through the full application process, income calculation worksheets, and what to expect after approval, including the 30-to-90-day lag before Social Security stops deducting the Part B premium.
