Kentucky Financial Help for Seniors on Medicare: Programs That Pay Premiums and Drug Costs
The Programs Most Kentucky Seniors Don't Know About
A senior living on Social Security in Kentucky has more financial help available for Medicare costs than almost any other category of expense — but participation rates tell a different story. Roughly 20% to 30% of eligible low-income Kentucky seniors and adults with disabilities remain unenrolled in SLMB and QI. These programs pay the Part B premium and trigger Extra Help; QMB also covers Medicare cost-sharing. The programs exist. The awareness doesn't.
Here's what's available, how the pieces fit together, and which one to apply for first.
Medicare Savings Programs: The Foundation
Kentucky's Medicare Savings Programs — QMB, SLMB, QI, and QDWI — are administered by the Department for Community Based Services and pay part or all of your Medicare costs depending on your income:
QMB covers Medicare Part A premiums, Part B premiums ($202.90/month in 2026), deductibles, coinsurance, and copayments. Individual income limit: $1,350/month after the $20 disregard.
SLMB and QI each cover the full Part B premium. SLMB goes up to $1,616/month; QI goes up to $1,816/month.
QDWI covers the Part A premium for disabled workers who lost SSI due to earnings. Income limit: $2,628/month.
All four have a resource limit of $9,950 individual / $14,910 couple (QDWI: $4,000 / $6,000). Your home, one car, and burial reserves don't count.
Applying through kynect.ky.gov, by phone at 1-855-306-8959, or with paper Form MAP-205 at your county DCBS office.
Extra Help: Automatic When You Get MSP
Approval for any Kentucky MSP automatically triggers deemed eligibility for Medicare Part D Extra Help (Low-Income Subsidy). No separate application needed — CMS receives the enrollment data from Kentucky and deems you eligible.
Extra Help in 2026 means: $0 monthly Part D premium when enrolled in a benchmark plan, $0 deductible, and drug copayments capped at $4.90 for generics and $12.15 for brand-name medications. Once your total out-of-pocket drug spending hits the $2,000 annual cap, copays drop to $0 for the rest of the year.
This automatic deeming is one of the strongest reasons to apply for MSP even if your income is closer to the SLMB or QI ceiling. The Part B premium payment alone saves $2,434.80 per year. MSP enrollment also triggers Extra Help, which reduces Part D costs.

KPAP: Not a Drug Plan, but Still Useful
The Kentucky Prescription Assistance Program sounds like a state drug benefit, but it's actually a navigation service. KPAP, run by the Department for Public Health, connects uninsured and underinsured Kentuckians with pharmaceutical manufacturer Patient Assistance Programs. If your parent takes an expensive brand-name medication that Extra Help doesn't fully cover, KPAP staff can help them apply for the manufacturer's free or discounted supply program.
KPAP doesn't pay pharmacy bills directly. It's a referral service — but a useful one if you know what it actually does.
Kentucky SHIP: Free Expert Counseling
The State Health Insurance Assistance Program provides free, unbiased Medicare counseling through local Area Agencies on Aging. SHIP counselors can help determine which programs your parent qualifies for, review enrollment options during open enrollment, and explain confusing notices from DCBS or CMS.
Call 1-877-293-7447. During the Annual Enrollment Period (October 15 through December 7), lines are busy — call early in the day or early in the season.
Where to Start
If your parent is on a fixed Social Security income and struggling with Medicare costs, apply for MSP first. It's the single application that unlocks the most downstream benefits — Part B premium payment, Extra Help for drugs, and QMB's balance billing protection against medical bills.
The Kentucky Medicare Savings Programs & Extra Help Guide consolidates every program's eligibility rules, application steps, and dollar calculations into one resource built specifically for Kentucky families.
