Kansas Medicare Savings Program vs Medicaid — What's the Difference?
They Share a KanCare Application — but the Rules Are Different
Both programs are administered under Kansas's KanCare umbrella, and both use the KanCare Clearinghouse for intake. That overlap leads families to assume they work the same way. They do not. The Medicare Savings Programs (QMB, LMB, ELMB) and long-term care Medicaid serve different purposes, have different financial thresholds, and carry very different consequences for assets.
Income and Resource Limits (2026)
| Rule | Medicare Savings Programs | Long-Term Care Medicaid |
|---|---|---|
| Income limit (single) | $1,350–$1,816/mo depending on tier | Special income cap or medically needy spend-down |
| Resource limit (single) | $9,950 | $2,000 |
| Resource limit (couple) | $14,910 | Complex spousal rules apply |
| $20 disregard | Yes | Yes |
| Primary home | Excluded at any value if the applicant lives there or intends to return | May be subject to estate recovery after death |
The resource gap is stark. A senior with $8,000 in savings qualifies for any MSP tier. The same $8,000 disqualifies them from long-term care Medicaid.
Estate Recovery
This is the difference families worry about most.
Medicare Savings Programs: Exempt from estate recovery. Kansas cannot pursue a claim against a deceased enrollee's home or other assets to recover premiums or cost-sharing it paid. This is federal law — 42 U.S.C. 1396p — and it applies regardless of the enrollee's age.
Long-term care Medicaid: Subject to estate recovery. If the beneficiary received nursing facility care, HCBS waiver services, or institutional care after age 55, Kansas can file a claim against their estate after death. The primary home is typically the largest asset at risk.
A parent enrolled only in QMB, LMB, or ELMB has no estate recovery exposure. That changes if they later enroll in full Medicaid for long-term care.

Lookback Period
Medicare Savings Programs: No lookback period. No asset transfer penalties. The Clearinghouse evaluates current resources on the first day of the application month, period.
Long-term care Medicaid: 60-month lookback. Gifts, transfers for less than fair market value, and large purchases within the five years before the application date trigger a penalty period of ineligibility.
What Each Program Pays For
Medicare Savings Programs pay Medicare's costs:
- Part B monthly premium ($202.90 in 2026)
- Part B deductible and coinsurance (QMB only)
- Part A hospital deductible and coinsurance (QMB only)
- Part A premium for those without premium-free Part A (QMB, if eligible, and QDWI)
Long-term care Medicaid pays for services Medicare does not cover:
- Nursing home room and board
- Home and community-based waiver services
- Personal care services
The two can overlap. A "full dual" beneficiary receives both MSP cost-sharing protection and Medicaid long-term care services. But many people qualify for an MSP without qualifying for long-term care Medicaid — and they should apply, because the estate recovery and lookback restrictions do not follow.
Why the Distinction Matters for Families
Many Kansas seniors avoid applying because they confuse the MSP with the Medicaid program their neighbor's family dealt with during a nursing home placement. The conversations are different. MSP enrollment does not require a Miller Trust, does not trigger a spend-down, and does not put the house at risk.
If your parent's countable monthly income after the $20 unearned-income disregard is at or below $1,816 and countable resources are at or below $9,950 (the single-person limits), they may qualify for an MSP tier that saves $200+ every month — with none of the asset restrictions families associate with "Medicaid."
