Kansas Extra Help for Medicare Part D — How to Get the Low-Income Subsidy
Two Ways Kansas Seniors Get Extra Help
Extra Help (also called the Low-Income Subsidy or LIS) lowers prescription drug costs under Medicare Part D. In Kansas, there are two routes to get it: one through an MSP without a separate SSA application, and one directly through SSA.
Route 1 — Automatic through KanCare: Every Kansas resident approved for QMB, LMB, or ELMB is automatically deemed eligible for full Extra Help. The state transmits weekly eligibility data to CMS, which triggers the subsidy without the enrollee filling out a separate form. This is the route most Kansas seniors should take if their income falls within MSP limits.
Route 2 — Direct SSA application: Seniors whose income or resources exceed MSP thresholds but fall under the Extra Help limits can apply directly through Social Security using Form SSA-1020 (online at ssa.gov or by calling 1-800-772-1213).
2026 Extra Help Income and Resource Limits
| Single | Couple | |
|---|---|---|
| Monthly income | ~$1,883 | ~$2,555 |
| Annual income | ~$22,590 | ~$30,660 |
| Countable resources | $16,660 | $33,240 |
These limits are higher than the MSP caps. A senior who earns too much for ELMB ($1,816/month single) may still qualify for Extra Help through the SSA direct path.
What Extra Help Pays in 2026
Under the Part D redesign from the Inflation Reduction Act:
- No plan deductible for full Extra Help recipients
- Nominal copayments only — typically $0 for most generic drugs and a few dollars for brand-name medications
- No coverage gap — the historic "donut hole" is eliminated
- $2,100 out-of-pocket cap applies to all Medicare Part D enrollees, but full Extra Help recipients almost never reach it because their copayments are so low
For a senior taking four to six prescriptions, the difference between paying standard Part D cost-sharing and receiving full Extra Help can be hundreds of dollars per month.

The MSP-to-Extra Help Connection
This automatic deeming is one of the strongest reasons to apply for an MSP even if a senior qualifies for LMB or ELMB — tiers that only cover the Part B premium. On top of saving $202.90/month in Part B costs, MSP approval unlocks full Extra Help for prescription drugs without a second application.
When a pharmacist looks up a newly approved MSP enrollee and the system has not updated yet, two emergency pathways resolve the delay:
- Best Available Evidence (BAE): Present the KanCare approval notice to the Part D plan. CMS rules require the plan to update copays within 24 to 48 hours.
- LI NET: A temporary federal program administered by Humana that provides immediate $0 copay prescription access for low-income beneficiaries not yet enrolled in a permanent Part D plan.
Kansas Has No SPAP to Fill the Gap
Some states run their own State Pharmacy Assistance Programs that layer additional drug coverage. Kansas does not. The historic "SeniorCare" program is defunct. For Kansas seniors, Extra Help through MSP deeming or the SSA direct application is the primary path to reducing drug costs.
Getting Started
If your parent's income is under the MSP limits, start with the KanCare application — it triggers both premium help and Extra Help in one filing. If income is too high for MSP but under the Extra Help thresholds, file SSA-1020 directly.
The Kansas Medicare Savings Programs & Extra Help Guide covers both paths, including what documentation the Clearinghouse needs and how to handle pharmacy delays after approval.
