Iowa SLMB and Expanded SLMB (QI) Program
Iowa seniors whose income runs slightly above QMB limits often assume they're out of options. They're not. Iowa operates two additional Medicare Savings Programs — SLMB and Expanded SLMB — that cover the Part B premium for beneficiaries earning up to 135% of the federal poverty level. That's $2,434.80 per year back in their pocket.
SLMB: 100% to 120% FPL
The Specified Low-Income Medicare Beneficiary program covers the standard Medicare Part B premium — $202.90 per month in 2026. It doesn't cover deductibles or coinsurance the way QMB does, but the premium relief alone is significant on a fixed income.
Iowa's 2026 SLMB income range runs from $1,330.01 to $1,596 per month for an individual ($1,804.01 to $2,164 for a couple) using published net limits. With the standard $20 monthly disregard applied to unearned income, the effective ceiling reaches $1,616 single and $2,184 couple.
Asset limits match the other MSP tiers: $9,950 individual, $14,910 couple, with the same exclusions for the primary home, one vehicle, burial funds, and irrevocable burial contracts.
SLMB coverage can be made retroactive up to three months before the application month, provided the beneficiary met all eligibility requirements during those months. This is a meaningful difference from QMB, which only starts the first of the month after approval.
E-SLMB: Iowa's Name for the Federal QI Program
What federal regulations call the "Qualifying Individual" or QI program, Iowa HHS calls Expanded SLMB (E-SLMB). The coverage is identical to SLMB — Part B premium only — but the eligibility band extends higher, from 120% to 135% FPL.
The 2026 income range is $1,596.01 to $1,796 per month single ($2,164.01 to $2,435 couple) before disregards, or effectively up to $1,816 single and $2,455 couple.
One important structural difference: E-SLMB operates under a capped annual federal allocation. Iowa HHS processes applications first-come, first-served, so apply as early as possible.
Beneficiaries cannot hold E-SLMB and full Medicaid simultaneously. If someone qualifies for Medically Needy spend-down, they can receive E-SLMB until the monthly spend-down threshold is met, at which point coverage converts to full Medicaid for that period.
Both Programs Trigger Extra Help
SLMB and E-SLMB approval automatically triggers deemed Extra Help (Low-Income Subsidy) for Part D prescription drugs — the same automatic benefit QMB provides. No separate application to Social Security is needed. That means $0 Part D deductible, $0 premium on benchmark plans, and, for beneficiaries qualifying through an MSP, 2026 copays capped at $5.10 for generic drugs and $12.65 for brand-name drugs; some full-benefit dual eligibles may pay less.
Deemed Extra Help status lasts through December 31 of the year, even if the MSP case closes mid-year. If coverage isn't restored by year-end, Extra Help expires unless the beneficiary reapplies through Iowa HHS or files a direct LIS application with SSA.

How to Apply
The application process is the same for SLMB and E-SLMB as for QMB. File through the Iowa HHS online portal at hhsservices.iowa.gov, by phone at 1-855-889-7985, or on paper using Form 470-5170. Iowa HHS determines which tier the applicant qualifies for based on calculated net income — you don't need to specify a particular program.
Iowa HHS has 45 calendar days to process the application, or 90 if a formal disability determination is required. After approval, the state transmits a buy-in record to CMS, which instructs Social Security to stop withholding Part B premiums. This adjustment takes 30 to 60 days. Premiums withheld after the effective date — including retroactive months for SLMB/E-SLMB — are refunded by SSA.
Renewals Work the Same Way
Both programs follow the standard 12-month redetermination cycle. Iowa HHS first attempts an automated renewal using federal databases. If that succeeds, the beneficiary doesn't need to do anything. If not, a paper renewal packet arrives with a 30-day return deadline.
The Iowa Medicare Savings Programs & Extra Help Guide includes income worksheets for all three MSP tiers and a renewal calendar to track the annual redetermination timeline.
