Illinois Medicare Savings Program Documents Needed

The Documents IDHS Actually Requires

The number-one reason Illinois Medicare Savings Program applications stall is missing or incomplete documents. When the Family Community Resource Center (FCRC) doesn't have what it needs, it sends Form HFS 267M — a verification request with a strict 10-calendar-day deadline. Miss that window and the application is denied automatically.

Here's what you need to gather before applying, organized by category.

Identity, Residency, and Medicare Proof

Medicare entitlement: A copy of the red, white, and blue Medicare card showing Part A coverage. If you don't have the card yet, a Social Security Award Letter or printout from SSA showing Medicare entitlement works. The caseworker verifies Part A through an electronic query (SOLQ/BENDEX), but having paper proof speeds processing.

Identity and citizenship: A U.S. passport, Illinois driver's license or state ID, birth certificate, or naturalization certificate. Qualified non-citizens need documentation of lawful permanent resident status with at least five years of qualified status, or refugee/asylee documentation.

Illinois residency: A utility bill dated within 60 days, a residential lease agreement, or a state property tax bill. The intent is to establish that you live in Illinois and plan to stay.

Income Documentation

IDHS counts gross income — before Medicare premiums, tax withholdings, or voluntary deductions are subtracted.

Social Security benefits: The most recent SSA-1099 (mailed every January) or a current benefit verification letter from SSA. If you receive both retirement and survivor benefits, document both.

Pensions: Formal pension statements showing gross monthly amounts. This includes state pensions (TRS, SERS, SURS), private pensions, VA benefits, and Railroad Retirement. The caseworker counts these at face value before applying the state's $25 monthly income disregard.

Earned income: If working, provide 30 consecutive days of paystubs. Self-employment income requires a federal tax return with Schedule C. Earned income gets more generous disregards — the first $65 plus half the remainder — but you still need to prove the numbers.

The Bank Statement Requirement

This is where most delays happen. IDHS requires 30 to 60 consecutive days of complete bank statements — every page, including ones that say "this page intentionally left blank."

The statements must cover every account: checking, savings, CDs, money market accounts. Joint accounts are presumed entirely yours unless you can prove the other person contributed their own funds and the account is restricted.

Retirement accounts (IRAs, 401(k)s, Keogh plans) are counted at their cash surrender value unless they're in periodic payment status. If your parent has retirement accounts, get a current statement showing the balance.

The asset limits are $9,950 for a single person, $14,910 for a married couple. Your home, one car, household goods, irrevocable burial contracts, revocable burial funds up to $1,500 per person, and dedicated burial spaces for you and immediate family are exempt. Bank accounts, CDs, stocks, and other countable resources count toward the limit.

What to Do When You Get Form HFS 267M

If any document is missing, the FCRC sends Form HFS 267M — "Instructions to Client" — listing exactly what's needed. You have 10 calendar days to respond.

Upload the documents through the ABE portal under "Manage My Case," fax them to the number on the form, or deliver them in person to the FCRC. Keep a confirmation of every submission — screenshot the ABE upload confirmation, save the fax transmission report, or get a date-stamped receipt at the office.

If you can't gather everything in 10 days, call the FCRC and request an extension before the deadline passes. If you do not respond or request an extension, the application is denied under Code 00. You can request a fair hearing within 60 calendar days of the denial notice; follow the appeal instructions on the notice.

Organizing Everything Before You Apply

The fastest applications are the ones that arrive complete. Gather all documents before submitting through ABE or dropping off Form HFS 2378M at the FCRC. The Illinois Medicare Savings Programs & Extra Help Guide includes a pre-formatted document checklist that walks through each requirement and helps you verify nothing is missing before the caseworker opens your file.
