Help Paying Medicare Part B Premium in West Virginia
The Part B Premium Problem
The standard Medicare Part B premium in 2026 is $202.90/month. Social Security deducts it automatically before your check arrives. On a monthly benefit of $1,400, that's about 14.5% of your income gone before you pay rent, buy groceries, or fill a prescription.
West Virginia runs three programs that pay this premium for you. All three are administered by the Department of Human Services and funded through a combination of federal and state dollars.
Three Programs, Three Income Tiers
QMB (Qualified Medicare Beneficiary) — up to $1,350/month single, $1,824/month couple: Pays your Part B premium plus all Medicare deductibles, coinsurance, and copays. The most comprehensive tier.
SLMB (Specified Low-Income Medicare Beneficiary) — $1,351–$1,616/month single, $1,825–$2,184/month couple: Pays your Part B premium. No cost-sharing coverage.
QI (Qualifying Individual) — $1,617–$1,816/month single, $2,185–$2,455/month couple: Pays your Part B premium. Funded through a capped federal grant, first-come basis.
All three tiers require resources below $9,950 (individual) or $14,910 (couple). Your primary home is excluded up to the $752,000 equity limit, and one vehicle is excluded regardless of value.
These income figures already include the $20 general disregard. A gross Social Security benefit of $1,636/month produces a countable income of $1,616 — right at the SLMB ceiling.
How the Premium Actually Stops
After West Virginia approves your MSP application, the Bureau for Medical Services sends your enrollment to the federal CMS Buy-In exchange. CMS updates your record with SSA, and SSA stops deducting the Part B premium from your monthly check.
This process takes 60 to 90 days. During that window, you'll keep seeing the $202.90 deduction. Don't panic — once the systems sync, SSA calculates every month of over-deduction back to your effective date and sends a lump-sum reimbursement by direct deposit or paper check.
If the deduction continues past 90 days, take your DoHS Notice of Decision to your local SSA field office and request a manual adjustment through their Buy-In problem resolution process.

SLMB and QI Can Backdate — QMB Can't
SLMB and QI eligibility can be retroactive up to three months before your application date. If you applied in October and qualified starting in July, you'll get three months of premiums reimbursed that you already paid.
QMB is strictly prospective — it starts the first day of the month after approval. No backdating, no matter how long you should have been covered. This makes filing promptly especially important for QMB applicants.
What Else You Get Automatically
Every MSP tier — QMB, SLMB, and QI — automatically qualifies you for Extra Help (the Low-Income Subsidy) with Part D prescription drug costs. No separate application needed. That means:
- $0 annual Part D deductible
- $0 premium on benchmark Part D plans
- Generic copays capped at $5.10 per fill
- Brand-name copays capped at $12.65 per fill
For someone taking three or four maintenance medications, Extra Help alone can save hundreds of dollars a month.
How to Apply
Apply through WV PATH at wvpath.wv.gov, mail Form DFA-QSQ-1 to your county DoHS office, or call 1-877-716-1212. You'll need your Medicare card, Social Security award letter, and a recent bank statement. The state has 30 calendar days to process your application.
If you're not sure which tier you qualify for, apply anyway — caseworkers evaluate you for all three tiers from a single application. You don't have to pick one.
The West Virginia Medicare Savings Programs guide includes an income worksheet that calculates your exact tier and a premium refund tracker for the waiting period after approval.
