Florida Low-Income Medicare Help: Programs That Reduce Premiums and Drug Costs

Living on Social Security in Florida means every deduction hits harder. The $202.90 monthly Part B premium alone takes 10–15% of a typical benefit check before it reaches the bank account. Add prescription copays, the Part B deductible, and hospital cost-sharing, and Medicare's "coverage" starts feeling like a second rent payment. Several programs exist to bring those costs down — the challenge is knowing which ones stack and how to access them.

Medicare Savings Programs (MSP)

This is the single highest-value program for low-income Medicare beneficiaries in Florida. The Department of Children and Families (DCF) runs three tiers:

QMB covers the Part B premium, Part A premium (if applicable), and all Medicare deductibles, coinsurance, and copayments. For a single person with countable monthly income up to $1,350 and resources under $9,950.

SLMB covers the Part B premium only. For income between $1,350 and $1,616 (single).

QI also covers the Part B premium only. For income between $1,616 and $1,816 (single). Funded through a capped grant — applications are first-come, first-served.

Couple limits are higher: $1,824 for QMB, $2,184 for SLMB, $2,455 for QI. The resource limit is $14,910 for couples across all three tiers.

Income is calculated after a $20 monthly disregard, and earned income gets additional deductions — so many seniors with income slightly above the published limits still qualify.

Apply through the MyACCESS portal, paper Form CF-ES 2282, or a local DCF service center.

Extra Help (Part D Low-Income Subsidy)

Extra Help reduces out-of-pocket costs for Medicare Part D prescription drugs. Once enrolled, copays drop to $1.60–$5.10 for generic drugs and $4.90–$12.65 for brand-name drugs. The Part D annual deductible disappears, and late enrollment penalties are waived.

One way to get Extra Help is through an MSP. Approval for any Medicare Savings Program — QMB, SLMB, or QI — automatically qualifies the beneficiary for Extra Help without a separate application. DCF sends the approval data to CMS, which updates the beneficiary's Medicare record.

For people who do not qualify for MSP, Extra Help has its own limits. For 2026, annual income must be below $23,940 for an individual or $32,460 for a married couple; resource limits are $18,090 for an individual and $36,100 for a couple when the burial exclusion applies. Apply through Social Security at ssa.gov or by calling 1-800-772-1213.

The Medicare Savings Account (MSA) — Not the Same Thing

One source of confusion: Medicare Savings Account plans are a type of Medicare Advantage plan with a high deductible and a savings account deposit. They have nothing to do with Medicare Savings Programs. The names are similar but the programs are entirely unrelated.

SHINE Counseling

The Serving Health Insurance Needs of Elders (SHINE) program provides free, unbiased counseling through the Florida Department of Elder Affairs. SHINE volunteers can explain eligibility, compare Part D plans, and confirm which MSP tier fits a particular income situation.

Contact SHINE through the Elder Helpline at 1-800-963-5337. Appointments are available by phone, at Area Agency on Aging offices, and at community sites across the state.

SHINE counselors cannot complete MSP applications, gather documents, or act as an authorized representative with DCF — but they are the best starting point for understanding what help is available.

Other Programs That Stack

Medicare Part D $2,100 out-of-pocket cap — effective in 2026 under the Inflation Reduction Act, everyone on Part D has an annual cap on out-of-pocket drug spending. This applies on top of Extra Help, giving dual protection against high drug costs.

Florida Community Care for the Elderly (CCE) — a state-funded program administered by the Department of Elder Affairs through Area Agencies on Aging, providing in-home and community-based services such as homemaking, personal care, and respite for functionally impaired elders. Eligibility is separate from MSP.

LIHEAP — federal energy assistance for low-income households. Does not affect MSP eligibility — energy assistance payments are excluded from income calculations.

SNAP/Food Stamps — also does not count as income for MSP purposes. Applying for food assistance through the same MyACCESS portal will not affect an MSP application.

Veterans Aid and Attendance — VA pension benefit for wartime veterans or surviving spouses needing help with daily activities. The VA pension amount is counted as unearned income for MSP purposes, but many VA pension recipients still fall within MSP income limits.

What Florida Does Not Have

Florida does not operate a general senior State Pharmaceutical Assistance Program (SPAP). Programs marketed as "Florida Rx Card" or similar are commercial discount card programs, not state-funded drug assistance. Florida's state-administered drug assistance is the Florida AIDS Drug Assistance Program (ADAP), which serves people with HIV; it is not a general senior SPAP.

This makes Extra Help (through MSP enrollment or separate application) the main broadly available prescription cost-reduction program for most low-income Medicare beneficiaries in Florida.

Where to Start

The highest-impact first step is applying for a Medicare Savings Program through DCF. If approved, everything else cascades: Extra Help enrolls automatically, the Part B premium stops being deducted from the Social Security check, and for QMB enrollees, all Medicare cost-sharing disappears.

The Florida Medicare Savings Programs & Extra Help Guide covers the complete MSP application with income worksheets, document checklists, and a timeline tracker from application through premium refund — organized for both seniors applying themselves and adult children managing the process from a distance.
