Financial Help for Family Caregivers: Programmes That Pay You or Offset the Cost
The Hidden Financial Drain of Caregiving
Long-distance caregivers spend roughly $8,728 per year out of pocket on caregiving-related expenses, compared with $5,885 for co-resident caregivers and $4,570 for caregivers living near the care recipient. The extra cost comes from emergency travel, time off work, and the premium cost of hiring local help from afar. These are not numbers from a survey of wealthy families paying for luxury care. These are averages that include skipped meals, deferred retirement contributions, and reduced work hours.
Most caregivers absorb these costs silently because they do not know that programmes exist to offset them. Several do — and some will actually pay a family member to provide care that would otherwise go to a hired aide.
US: Medicaid Self-Direction Programmes
In most states, Medicaid HCBS waiver programmes include a "self-directed" or "consumer-directed" option that allows the care recipient to hire their own caregiver — including, in many states, a family member. The caregiver is paid under the waiver's state-specific rate and care-plan rules.
The rules are state-specific and narrow:
- The care recipient must already be enrolled in a Medicaid HCBS waiver programme (means-tested, with the standard $2,000 asset and $2,982/month income limits)
- Whether a spouse or adult child may be paid is state-specific — confirm the rule with the waiver administrator
- The family caregiver must pass a background check and may need to complete a training programme
- A fiscal intermediary handles payroll, taxes, and workers' compensation
- The caregiver's hours and tasks are defined by the care plan — you cannot be paid for general companionship
Contact the local Area Agency on Aging to find out whether your parent's state offers a self-directed option and whether family members qualify.
US: Veterans' Aid and Attendance
If your parent is a wartime veteran (or the surviving spouse of one), the VA Aid and Attendance pension supplement provides a monthly benefit to help pay for care. The amount depends on the VA's current Maximum Annual Pension Rate, household and dependent status, income, and deductible medical expenses; check the current VA rate table.
Eligibility requires:
- Active-duty service during a wartime period (WWII, Korea, Vietnam, Gulf War, post-9/11)
- At least 90 days of active duty, with at least one day during a wartime period
- A medical determination that the veteran needs regular assistance with ADLs or is housebound
- Financial eligibility based on net worth and income thresholds
To apply, the caregiver must obtain the veteran's DD Form 214 (discharge papers). If these are not in the family's possession, submit Standard Form 180 (SF-180) to the National Personnel Records Center (NPRC) in St. Louis. For urgent situations (active hospitalisation or hospice), fax the SF-180 to the NPRC emergency line at 314-801-0764.

US: FMLA Job Protection
The Family and Medical Leave Act provides up to 12 workweeks of unpaid, job-protected leave within a 12-month period to care for a parent with a serious health condition. FMLA does not pay you, but it protects your job and benefits while you take time off for caregiving.
Eligibility requirements: your employer must have 50 or more employees within a 75-mile radius, you must have worked there for at least 12 months, and you must have logged at least 1,250 hours of actual work in the preceding 12 months (PTO and sick leave do not count toward the 1,250 hours).
FMLA leave can be taken as a continuous block or intermittently — a day here, a few hours there — when medically necessary. Intermittent leave is particularly useful for long-distance caregivers who need to travel for medical appointments, emergency hospitalisations, or care transitions.
A critical detail: FMLA protections apply to employees physically working in the US. If you are assigned by a US company to work abroad on a long-term contract, FMLA does not cover you. But if you work in the US and your parent lives abroad, you are legally entitled to take FMLA leave to travel to their country for care — and your employer must accept medical certifications from healthcare providers in that country.
US: Tax Benefits
Medical expense deduction: If you provide more than half of your parent's financial support and can claim them as a dependent, you may deduct unreimbursed medical expenses (including in-home care, if medically necessary) that exceed 7.5% of your adjusted gross income.
Dependent care credit: In limited circumstances, if your parent qualifies as a dependent and you pay for their care so that you can work, you may be eligible for the dependent care credit. The qualifying conditions are strict — the parent must live with you for more than half the year and be unable to care for themselves.
Canada: Canada Caregiver Credit
A non-refundable federal tax credit for supporting a dependent with a physical or mental impairment. For the 2026 tax year, the maximum claimable amount for a parent (claimed on Line 30450 of the T1 return) is $8,773, reducing the federal tax bill by up to $1,316 (15% of the claim amount).
The credit phases out as the parent's net income rises above $20,601 and is fully eliminated at $29,374. A physician's statement documenting the nature and duration of the impairment is required, unless the parent has an approved Form T2201 (Disability Tax Credit Certificate) on file with the CRA.
Canada: EI Family Caregiver Benefits
Employment Insurance provides up to 15 weeks of benefits — paying 55% of average insurable earnings to a maximum of $729 per week in 2026 — for caregivers who take time off work to care for a critically ill adult family member. The caregiver must have accumulated at least 600 hours of insurable employment during the qualifying period, and a physician must certify that the family member is at significant risk of death within 26 weeks.
UK: Carer's Allowance
A weekly payment of £86.45 (2026/27 rate) for caregivers who provide at least 35 hours of care per week to someone receiving a qualifying disability benefit (Attendance Allowance, Personal Independence Payment, or similar). The carer's earnings must not exceed £204 per week after deductions. This is not compatible with most full-time employment, but it provides a baseline income for caregivers who have reduced their work hours significantly.
Australia: Carer Payment and Carer Allowance
Carer Payment: An income-tested fortnightly payment (up to $1,200.90 per fortnight for a single person at the current 2026 rate) for caregivers who provide constant care. A recipient may still qualify while working up to 100 hours in a 4-week period. This is a substantial benefit, but the "constant care" requirement is difficult to meet for long-distance caregivers.
Carer Allowance: A supplementary payment of $162.60 per fortnight, not income-tested, for caregivers who provide daily care to someone with a disability or medical condition. This can be claimed in addition to the Carer Payment or on its own.
What Long-Distance Caregivers Should Do First
Start with the programmes that do not require you to be physically present: Medicaid self-direction (if your parent qualifies), Veterans' Aid and Attendance (if applicable), and the relevant tax credits. FMLA leave protects your job when you need to travel for care.
