Connecticut Extra Help Medicare Part D — Automatic Qualification Through MSP

One of the most valuable things about qualifying for Connecticut's Medicare Savings Program is something many families never realize: enrollment in any MSP tier — QMB, SLMB, or ALMB — automatically qualifies the beneficiary for Full Extra Help on Medicare Part D prescriptions. No separate application required.

How Automatic Deeming Works

When the Connecticut Department of Social Services approves someone for QMB, SLMB, or ALMB, that data gets transmitted to CMS. CMS then automatically deems the person eligible for the federal Low-Income Subsidy, known as Extra Help. The beneficiary does not need to file a separate application with the Social Security Administration.

This matters because standalone Extra Help applicants who apply directly through SSA face federal income and asset tests — $9,950 for individuals and $14,910 for couples in 2026. Connecticut residents who qualify through MSP bypass those federal resource limits entirely, thanks to the state's elimination of its own asset test under CGS § 17b-95.

What Full Extra Help Covers in 2026

Full Extra Help provides substantial prescription drug savings:

  • $0 monthly premium for benchmark Part D plans
  • $0 annual deductible on covered prescriptions
  • Reduced copayments — nominal amounts for covered drugs
  • No coverage gap — full protection from the Part D donut hole
  • Special Enrollment Period — ability to switch Part D plans once per quarter during the first three quarters of the year (January through September)

The 2026 Inflation Reduction Act reforms cap all Part D out-of-pocket spending at $2,100 annually for everyone. For MSP enrollees with automatic Extra Help, actual drug costs stay well below that cap because of the additional LIS subsidies.

When the Pharmacy Does Not Recognize Extra Help

There is a processing delay between DSS approval and when the pharmacy's CMS database reflects Extra Help status. If the pharmacy rejects the LIS subsidy:

Present Best Available Evidence. Show the pharmacist a copy of the DSS MSP Approval Notice (Form W-409) or a state Medicaid card. CMS rules require pharmacies to update copay amounts immediately when presented with acceptable proof of eligibility.

Ask about LINET. The Limited Income NET program, administered by Humana under contract with CMS, provides temporary prescription drug coverage at the pharmacy counter for low-income beneficiaries whose permanent plan enrollment is still processing. LINET eliminates upfront costs while the system catches up.

How an Extra Help Application Can Trigger MSP Screening

The Medicare Improvements for Patients and Providers Act created a data-referral pathway. When a Connecticut resident applies to SSA for standalone Extra Help, SSA electronically transmits that application data to the Connecticut Department of Social Services. Under current state policy, DSS treats this electronic transfer as an affirmative MSP application — establishing the filing date as the date the LIS request was submitted to SSA.

This means someone who applies for Extra Help through Social Security may get screened for Connecticut's MSP at the same time, potentially qualifying for Part B premium help they did not know existed.
