Conditional Part A in Kansas — The Group Payer Process for Seniors Without Work Credits
If your parent doesn't have 40 quarters of work credits, they don't get premium-free Medicare Part A. The monthly premium is $311 (30–39 quarters) or $565 (under 30 quarters) in 2026. For a senior on a fixed income, that's devastating — and it's the exact situation where QMB can help, because QMB pays the Part A premium. But getting there in Kansas involves a specific two-step process that most SSA offices don't volunteer.
Why Kansas Is Different: Group Payer Status
States fall into two categories for Part A enrollment:
- Buy-In states allow conditional Part A applications year-round
- Group Payer states restrict conditional applications to the General Enrollment Period (January 1 – March 31)
Kansas is a Group Payer state (SSA POMS HI 00801.140, Section D). This means your parent cannot walk into a Social Security office in July and apply for conditional Part A. They must wait for the GEP window, apply at SSA, then separately apply for QMB at the KanCare Clearinghouse.
The Two-Step Process
Step 1: Apply at Social Security (January 1 – March 31)
Your parent visits or calls their local Social Security office during the GEP and requests a conditional application for Premium Part A using Form SSA-18-F6.
The critical part: the SSA representative must include a specific statement in the remarks section of the application: "The individual is applying for Premium Part A conditionally, subject to a determination of eligibility for QMB coverage by the State Medicaid agency."
Without this conditional language, SSA may process the application as a standard Part A enrollment — and your parent receives a monthly bill for $311 or $565 instead of having QMB pay it.
If the SSA representative is unfamiliar with conditional Part A or refuses to process the form, reference SSA POMS HI 00801.140 Section D by name. You can also call SHICK (Senior Health Insurance Counseling for Kansas) at 1-800-860-5260 for help escalating with the local office.
SSA will issue a conditional receipt letter confirming the application.
Step 2: Apply for QMB at KanCare
Take the conditional receipt letter from SSA and attach it to Form KC-1500 (Medical Assistance Application for the Elderly and Persons with Disabilities). Submit the application to the KanCare Clearinghouse:
- Online: applyforKanCare.ks.gov
- Mail: KanCare Clearinghouse, P.O. Box 3599, Topeka, KS 66601
- Fax: 1-844-264-6285
- Phone: 1-800-792-4884
The Clearinghouse processes the QMB application. If approved, Kansas adds your parent to the Group Payer roster through the Third Party Exchange (TPX) network with CMS. Part A coverage activates, and the state pays the premium directly — your parent never receives a Part A premium bill.
Timing Matters
Under the BENES Act, GEP coverage now starts the first month after enrollment. Previously, someone applying in February would wait until July 1 for coverage. That gap is gone — but the GEP window itself (January through March) still applies in Group Payer states like Kansas.
If your parent misses the GEP, they must wait until the next January to start Step 1. Use the waiting period to gather income and asset documentation for the QMB application so the KC-1500 is ready to submit immediately after the SSA conditional receipt arrives.

What If Your Parent Already Has Part B But Not Part A?
This is common for people who enrolled in Part B through their spouse's employer coverage but never had enough individual work credits for premium-free Part A. The conditional Part A process works the same way — the Part B enrollment isn't affected. Your parent keeps their Part B coverage and adds conditional Part A on top of it.
