Colorado Extra Help Low Income Subsidy
A parent paying $80 a month for a Part D plan premium and facing $200 copays at the pharmacy counter may not realize that a single state application could eliminate most of those costs. Extra Help — the federal Low-Income Subsidy for Medicare Part D — is one of the most valuable and least understood benefits available to low-income seniors in Colorado.
What Extra Help Covers
Extra Help (also called LIS — Low-Income Subsidy) reduces or eliminates three categories of Part D costs:
- Plan premiums: Full Extra Help pays the premium for any Part D plan priced at or below the regional benchmark. In 2026, everyone who qualifies for LIS receives the full subsidy.
- Annual deductible: Eliminated or sharply reduced.
- Copayments: In 2026, LIS copayments for covered medications are generally capped at $5.10 for generics and $12.65 for brand-name drugs; some full dual-eligibles in long-term care pay $0.
For someone taking three or four maintenance medications, Extra Help can save $3,000-$5,000 per year beyond what the Part D plan itself covers.
The Automatic Trigger From Colorado's MSP
Here's the piece most families miss: if Colorado approves your parent for any Medicare Savings Program — QMB, SLMB, or QI-1 — that approval automatically triggers deemed eligibility for full Extra Help. No separate application to SSA. No additional paperwork. HCPF sends the MSP eligibility data to CMS, which grants the LIS benefit.
This means applying for a Medicare Savings Program through Colorado PEAK or a county office can lead to both benefits: Part B premium relief from the MSP and prescription cost relief from deemed Extra Help eligibility.
Extra Help Eligibility on Its Own
Not everyone who qualifies for Extra Help also qualifies for an MSP. Extra Help has higher income and resource limits:
- Individual: Income up to $2,015/month, resources up to $17,600
- Couple: Income up to $2,725/month, resources up to $35,130
Compare that to the QI-1 ceiling of $1,816/$2,455 for MSP eligibility. A senior with income between $1,817 and $2,015 wouldn't qualify for QMB, SLMB, or QI-1 based on income, but could still qualify for Extra Help directly.
For those who don't qualify for an MSP, apply for Extra Help separately through SSA — either online at ssa.gov, by calling 1-800-772-1213, or at a local Social Security office. Under the MIPPA referral process, SSA also automatically sends application data to HCPF, triggering an MSP evaluation at the state level even if the senior only applied for Extra Help federally.

The MIPPA Referral Loop
Under the Medicare Improvements for Patients and Providers Act (MIPPA), when someone applies for Extra Help through SSA, the application data is electronically transferred to Colorado's HCPF. CBMS opens an MSP application record, and the county contacts the applicant to collect any remaining documentation needed for a state eligibility determination.
This creates a useful loop: applying for MSP through Colorado gets you Extra Help automatically if approved for MSP. Applying for Extra Help through SSA gets you evaluated for MSP by the state. Either entry point can lead to both benefits.
What to Do If Your Parent Qualifies
If your parent's income and resources fall within MSP range, apply through Colorado PEAK — one application, two benefits. If their income is above MSP limits but under Extra Help limits, apply through SSA directly and let the MIPPA referral handle the state evaluation.
Our Colorado Medicare Savings Programs & Extra Help Guide walks through both application paths, the income calculation that determines which programs apply, and how to verify that Extra Help has been properly activated after MSP approval.
