CHSLD Placement Process Quebec: Waitlist, Admission, and Costs

Your parent's CLSC case manager just said the words you've been dreading: "home care is no longer sufficient." Now you need to understand how public long-term care placement actually works in Quebec — the waitlist, the timeline, the costs, and the decision window you'll face when a bed opens up.

Who Qualifies for a Public CHSLD

A Centre d'hebergement et de soins de longue duree (CHSLD) provides 24/7 medical care and supervision for seniors with severe physical or cognitive impairment. To qualify for a public CHSLD bed, your parent must:

• Have an ISO-SMAF profile of 10 or higher (severe loss of autonomy)
• Have a completed OEMC assessment from the CLSC confirming their needs exceed what home care can safely deliver
• Have a physician complete the CTMSP medical evaluation form confirming the clinical need for long-term care

Profiles 10–14 represent the most significant functional loss: advanced dementia with wandering (Profile 10), near-total physical dependence (Profile 13), or conditions requiring continuous medical supervision.

The MAH Waitlist

The Mecanisme d'acces a l'hebergement (MAH) is the regional waitlist that controls all public residential placements — CHSLDs, intermediate resources (RIs), and family-type resources (RTFs).

How it works:

1. Your parent's CLSC case manager updates the OEMC assessment and submits a placement request to the MAH
2. A physician completes the CTMSP form
3. The regional admission committee (Comite d'orientation-admission) reviews the file and assigns a priority level based on clinical urgency, not personal preference
4. Your parent is placed on the regional waitlist

Wait times vary dramatically by region:

• Montreal and urban centres: One to five years for a public CHSLD bed
• Smaller cities (Quebec City, Sherbrooke, Gatineau): Six months to two years
• Rural regions: Often shorter, sometimes weeks to months

These timelines are why starting the CLSC process early — even while your parent is stable at home — is critical. The waitlist is not first-come, first-served; clinical urgency determines priority. But having an active file with a current assessment means your parent is already in the system when a crisis hits.

The 24-to-48-Hour Decision Window

When a public CHSLD bed opens, the MAH contacts the designated representative. You have 24 to 48 hours to accept a CHSLD offer, and up to 72 hours for an intermediate resource (RI).

If you refuse the offer without documented clinical reasons — for example, the facility is in a different region than you wanted — the MAH may close the placement request entirely. You would need to restart the process.

This is one of the most stressful moments in the entire elder care journey. Families who haven't researched their regional facilities in advance are forced into a decision about their parent's final home within two days.

Public CHSLD Room Rates (2026)

Public CHSLDs charge regulated room and board rates on a sliding scale based on income and room type:

RAMQ Form 4473 — if your parent cannot afford the standard rate, the representative must submit Form 4473 (Application for Exemption or Re-Evaluation) along with Form 3906 (Identity of Representative) to RAMQ.

RAMQ evaluates the ability to pay based on:

• Primary residence exemption: up to $389,677 of home equity excluded if a spouse still lives there
• Liquid assets limit: $2,500 excluded for a single person ($5,000 for a couple)
• Vehicle exemption: up to $10,000 excluded
• Spousal support: $1,512/month excluded for a non-accommodated spouse
• Personal expense allowance: $349/month excluded for the resident's personal needs

Submit Form 4473 immediately upon admission. RAMQ typically processes reduction requests within 30 to 60 days, during which the resident pays a provisional rate.

Private CHSLDs: The Faster Alternative

Private, non-contracted CHSLDs do not require a CLSC assessment or MAH waitlist. Families can apply directly and may secure a bed within days to weeks. The tradeoff: full market rates of $3,500 to $8,000+ per month, with no RAMQ sliding-scale reduction.

Some families use a private CHSLD as a bridge — placing their parent in a private facility immediately while remaining on the public MAH waitlist for a subsidized bed.
