A diabetes care plan built around verbal reminders, written schedules, and self-monitoring doesn't survive contact with dementia. Bedtime blood sugar check is clinically important because overnight hypoglycemia is dangerous and often silent in dementia patients. Follow the physician's individualized plan for what to do with the reading rather than using a blanket bedtime threshold or snack rule.

Using Technology That Doesn't Require Participation

The biggest advantage of a continuous glucose monitor for a parent with dementia is that it removes the most confrontational moment from the daily routine: the finger stick. A CGM sensor is applied about every 10-15 days (depending on the system), and after that, blood sugar readings stream continuously to a smartphone app.

  • Behavioral episodes: agitation, wandering, repeated questions, refusal of care. Note the time, what happened before the episode, and how it resolved

  • Sleep quality: poor sleep increases insulin resistance and worsens cognitive function the next day

After two weeks of logging, patterns usually emerge. Share this log with your parent's physician. It gives the care team data they can't get from a 15-minute office visit, and it often leads to medication adjustments that reduce both hypoglycemic episodes and behavioral disturbances.

Handling Medication Resistance

A parent with dementia may refuse medication because they don't understand what it is, because they don't recognize you as someone authorized to give them pills, or because they're experiencing a paranoid delusion about being poisoned. None of these scenarios respond well to rational explanation.

Practical approaches that reduce resistance:

  • Pair medication with food: when the prescription allows it, offer pills with a suitable food rather than presenting them in a pill organizer
  • Simplify the regimen: ask the physician about deintensifying or simplifying it. The Beers Criteria flags sulfonylureas like glyburide and glimepiride as high-risk for elderly patients. A physician-directed simplification can reduce the daily pill burden and hypoglycemia risk simultaneously
  • Try again in 20 minutes: if your parent refuses, step away. Come back later and offer again without referencing the earlier refusal. Short-term memory loss means they may not remember saying no
